    To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: USDA Program Discrimination Complaint Form from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

  • Collegiate charges the following prices in our school cafeteria:

    FULL PRICE: Breakfast $2.50 Lunch $4.00

    REDUCED: Breakfast $.30 Lunch $.40

    If your student will be paying full or reduced price, you must deposit money into his/her cafeteria account if you plan for him/her to eat in the Collegiate cafeteria. Students with accounts that are more than $10.00 overdrawn are not allowed to charge any additional meals in the Collegiate Cafeteria, and a hold will be placed on their NetClassroom account. Parents can check student balances by clicking on the K12 payment center that is located on the CSM website or by calling Ms. Randolph at 901-591-8200.

    In addition to breakfast and lunch meals, Collegiate will also offer select à la carte items for purchase during lunch. Á la carte items may be purchased with cash only. Students must purchase a cafeteria lunch meal or bring a lunch meal from home in order to purchase à la carte items. Á la carte items cannot be a student’s full lunch.

  • Once we have received your application, we are committed to reviewing it within 30 days and promptly notifying you of the outcome. You will receive a notification through your preferred email account on record (via Blackbaud), delivered by Ms. Randolph.

  • You will need your scholar’s ID number which can be found in your scholar’s Blackbaud account.

  • No, you do not need your scholar’s student ID to apply.

  • Your account will not be active until you have verified your password. Check your email for the verification link.

  • There are two ways to receive your scholar’s balance. The first way is to set up an account on the K12 payment center. The second way is to email Ms. Randolph at arandolph@collegiatememphis.org with your scholar’s name in the subject line of the email.
